As a bouncer at a music venue, I would have to say that is an almost fool proof fake ID. Your best bet is to pay the Prof $500, print out the ID, and rubber cement it onto your high school ID. If you don't have a high school ID (hey lets face it, you weren't bright enough to avoid asking for a fake ID on a public forum that is probably routinely monitored by authorities so you might not have made that giant leap in education) try gluing it to an expired gift card or a Kroger discount card.
Later on when the bouncer confiscates your ID and laughs at you, you should try to physically retrieve the ID back. Bouncers love this and will probably let you in afterward because they admire your gumption.
